12/22/2023 EnchantaVaughn Approved 1. Behavior: Upon arrival to the home setting, QMHP-C was greeted by Ms. Hodge. While engaging in conversation with Ms. Hodge about updates on Ashiyah’s behaviors, it was discussed Ashiyah has to be redirected within the home as it relates to her impulsive behaviors (i.e. use of profanity and storming out the home when she does not have her needs meet (things going her way). 2. Behavior: QMHP-C entered Ashiyah’s appeared to be in a positive mood evident by her facial expressions such as a smile on the face. Ashiyah engaged in the session with QMHP-C and did not need any redirection. 1. Intervention: QMHP-C discussed her mood and learning to cope with the sadness, being unhappy. QMHP-C engaged Ashiyah in a discussion surrounding positive self-talk. QMHP-C informed Ashiyah the purpose of this discussion was to assist her with reducing negative thoughts and decision-making. QMHP-C then processed with Ashiyah on the handout. QMHP-C talked with Ashiyah on how her impulsive behaviors triggers negative consequences for her actions. QMHP-C engaged Ashiyah in coping skills to include thinking before acting, thought-stopping skills. 2. Intervention: QMHP-C taught Ashiyah that in any case thinking of acceptable and unacceptable behaviors before reacting supports dealing with impulsive behaviors. QMHP-C modeled effective ways for Ashiyah to communicate with authority figures, and peers. QMHP-C utilized role-rehearsal with Ashiyah to model ways to show respect to authority figures and learning to use anger management skills when becoming upset and frustrated with others. QMHP-C employed active listening skills as Ashiyah was able to converse about her angry feelings and acting out behaviors toward others whether authority figures in the school, and/or peers. 1. Response: Ashiyah appeared to be receptive to QMHP-C’s interventions and services provided to her during the session. Ashiyah has difficulty with expressing her feelings and thoughts age-appropriately. However, when engaged her in a discussion on positive self-talk, Ashiyah would answer questions and when confronted about negative behaviors Ashiyah denies her negative interactions and behaviors. Ashiyah hunches her shoulders when she refuses to take responsibility for her actions. Ashiyah indicated that she does not think about the consequences of her actions. 2. Response: Ashiyah was able to demonstrate to QMHP-C effective communication skills when expressing her thoughts and feelings about certain situations that occur within the community and home setting; however, she continues to be able to explain to QMHP-C why things make her so upset and why she is not able to handle herself in an assertive manner.
